List menu > Templates. Double-click those you use to Edit one. In Basic Customization, bottom left, remove checkmarks for fields you don’t want to include. Or, disable all of them, then Hardcode what you wanting using Layout Designer view, Add Text Box. Position it, input the text, change Properties, etc.
