correct template: I need to add customize fields to customer information form

Here’s how:

  1. Go to the Gear icon.
  2. Select Custom Form Styles.
  3. Click the correct template.
  4. Click the Content tab.
  5. Click the first box from the right.
  6. Click +Custom field under Display. From there, enter the name in the box. You can have up to 3 custom fields with a limit of 30 characters per field.
