I tried a third system using Firefox and there was no problem signing into QBO, everything displayed as expected.  So I went home and tried once again to clear everything on both LapTops with no success, but at least I was sure now it was not a QBO problem.  So […]

Yes, from the Reports menu pick Lists and then something like Customer Contact.  Modify it to show the fields of data you want to appear.

That report is showing the data in the payroll part of this data file. If something is wrong in the form, it is Wrong in payroll, because that is the source of the data.

Have you run the Payroll Summary Report for this quarter, to examine what went wrong?
